The My Sophi™ Difference

My Sophi allows people the ability to self-manage their health - to take back the controls - through daily monitoring of their health status.

My Sophi remote patient monitoring provides daily monitoring of patients’ health so that their caregivers, doctors, and family members have the ability to proactively intervene as soon as a problem with that person’s health occurs. More complete care decisions can be made based on this data versus the patient’s recollection.

My Sophi remote patient monitoring collects and sends patient generated data such as blood pressure, blood sugar level, oxygen saturation, temperature, and heart rate.

It is easy for patients to use with no installation required and simple set-up directions that are provided for each patient individually to ensure understanding and proper use.

Patient daily measurements are sent to our Care Center (never an outsourced call center) which is managed by RNs and staffed by locally licensed nurses and medical assistants.

In addition to daily vitals, Care Center staff use the My Sophi 2-way communication hub to send daily medication reminders or appointment reminders, provide educational information and act as a source of 2-way direct communication with the patient. Caregivers and family members, when programmed into the hub, can also communicate directly via the hub.

The My Sophi mobile app keeps caregivers/family connected with vitals reports, and messaging. They can check to see if mom took her medications that morning, weighed, checked her blood pressure and other vitals as related to her specific needs. They can also message directly to the hub.
