Recurring Monthly Credit/Debit Card Authorization Form

Please fill out all fields. Your credit/debit card information will be securely stored for recurring monthly payments for your My Sophi™ Services. You may cancel this authorization at any time by phone at 1-866-697-6744. This authorization will remain in effect until cancelled. PLEASE NOTE: You will be asked once more and required to input your credit/debit card information for the Initiation Charge associated with your My Sophi™ services upon selecting your devices and CHECKING OUT. Your card will not be charged when you input its information here on the Enrollment Form.
